As a part of the Haemovigilance in Denmark scheme, the Danish Society of Clinical Immunology wishes to monitor the frequence of serious transfusion hazards.

To this purpose,
the persons in charge of the blood banks are asked
to submit information on actual transfusion hazards
to the regional transfusion centre.

Submssion is anonymous and on a voluntary basis.

However, in order to make the clearest picture possible of the risk of blood transfusion, it is important that all hazards are reported and described as carefully as possible.

The forms can be e-mailed or otherwise delivered to

Submitted cases are treated confidentially.
When edb-registration has taken place, all written material concerning a case registered in DART will be destroyed.
Submissions can be stored locally by the submitter. In such cases copies should me made prior to submission.
